This is the basic outline you must use to have a chance of getting mod.

Username:

How long you have been playing:

Why you deserve to be a part of the staff:

Do and will you respect all rules and fellow people + staff members:

100% Respect everyone staff or not 100%:

What is your time zone?

When are you usually on?

Some people act like they are a experienced staff member but really arn't. Have you ever been staff on a server? If so whitch ones. If not, why?

You could add age as some information needed in the application, obviously if someone is 8 years old, you wouldn't want a dumb ass being a mod and i'm not being some stereotypical bastard but that's basically the truth, but obviously people are going to lie about their age which means it's not going to really work, but then, if someone says they are 15 and act like a 12 year old, you are obviously going to know. You could also add in where they live so that you know what times they can get onto the server.
